Home health patients receive the best care when your doctor and our agency work together. If you think that you would benefit from homecare service, talk to your physician or case manager if you are in the hospital.

If your physician feels you need home care, ask the doctor to contact us. We will quickly set an appointment to see you at your home. The nurse will assess the appropriateness of home health services and begin development of a care plan.

As a certified home care agency, PrimeCare is an approved provider for Medicare, Medicaid, Private Insurance and Worker's Compensation. We also accept most other insurances and will work directly with your insurance carrier for billing and paperwork.

If you are a Medicare Patient, the Registered Nurse and your physician will establish your homebound status and medical necessity by conducting a thorough medical assessment. We will then provide this information and a customized care plan to your physician for review and approval.
